If you received a profile from a H2F provider then you should follow-up with that provider. Profile extensions require a re-evaluation so please don't wait until the expiration day to follow-up with your provider. In most cases, follow-up appointments are booked so that there are not gaps in the care we provide you and things like profile extensions get taken care of.
